What a major vaginal repair instrument set is
A major vaginal repair instrument set is the complete tray for surgery done through the vagina rather than the abdomen. Google’s own summary lists what it covers: anterior and posterior colporrhaphy, pelvic floor reconstruction and vaginal hysterectomy, with the retraction, strong clamping and deep pelvic access those operations need. Ours is built for exactly that list.
Sixty three pieces sounds like a lot until you look at what vaginal surgery demands. The field is a narrow tunnel, so exposure needs several specula and retractors of different lengths. The tissue is thick and vascular, so the clamps come in fours. And the bladder sits just in front, so the tray carries a metal catheter to find it and fistula hooks to work round it.
Google names Deaver retractors in some versions of this tray. We carry Martin and Simon retractors instead, which are the patterns made for vaginal work, and a Gelpi for the perineum. Units that also want a Deaver can add one to the order.
This is our largest gynecology set, at 589.05 dollars and 4.77 kg with the box. A hospital that does pelvic floor surgery every week usually needs two, because a tray this size takes a full washer and autoclave cycle to turn round.
Key design features
Five specula and four retractors
An Auvard weighted speculum, 80 by 38 mm, holds the posterior wall. Two Doyen specula at 55 by 45 mm, Fig.1, and one at 85 by 45 mm, Fig.2, slightly concave, lift the anterior wall. A Sims vaginal speculum, double ended, 70 by 30 and 35 mm, gives two blade sizes in one instrument. Martin, Simon and Gelpi retractors finish the exposure.
Heavy clamps in fours
Four Heaney forceps with light jaws and four with two teeth, all 21 cm, the clamp most vaginal surgeons reach for on a pedicle. Anyone ordering Heaney forceps, curved, gets four with two teeth in this box. Four Mikulicz peritoneal clamps, 20.5 cm, extra strong, and two Ochsner-Kocher forceps, 20 cm, carry the heaviest tissue.
Tenacula for the cervix
Two Schroeder tenaculum forceps with box locks, 25 cm, and four Museux tenaculum forceps, 24 cm with 6 mm jaws. The Museux grips harder and is the one used when the cervix has to be drawn down.
Fistula hooks and a catheter
Two Simon fistula hooks, 22 cm, two prongs, hold the edges of a fistula or a flap while it is freed. The female metal catheter, 15 cm, 4 2/3 mm, marks the bladder and urethra so the dissection stays off them.

Where the set is used
Every operation below is done through the vagina, which is the reason this tray is shaped the way it is.
| Operation | What the set does | Pieces doing the work |
|---|---|---|
| Anterior colporrhaphy | Repairs a cystocele through the front wall | Doyen specula, catheter, Mosquito forceps, Metzenbaum |
| Posterior colporrhaphy and perineorrhaphy | Repairs a rectocele and the perineum | Auvard speculum, Gelpi retractor, Mayo scissors |
| Vaginal hysterectomy | Removes the uterus without an abdominal incision | Heaney forceps, Museux tenacula, Martin retractors |
| Fistula repair | Frees and closes a tract between bladder and vagina | Simon fistula hooks, catheter, Sims speculum |
| Vault and cuff repair | Closes or supports the top of the vagina | Simon retractors, needle holders |
What it does, and what it must not be asked to do
Big clamps in a small space. The limits below keep them from doing harm.
| Instrument | What it does | What it must not do |
|---|---|---|
| Heaney forceps | Clamp vascular pedicles before they are tied | Clamp the ureter. Stay close to the uterus when clamping a pedicle |
| Mikulicz clamps | Grip peritoneum and the heaviest tissue | Be used on thin vaginal mucosa. The 1×2 teeth tear it |
| Museux tenacula | Draw the cervix down firmly | Stay clamped on a soft or thin cervix for the whole case |
| Simon fistula hooks | Hold fistula edges and flaps | Lever against bone. The prongs bend |
| Female metal catheter | Marks the bladder and urethra | Be forced. A metal catheter can pass through a thin bladder wall |
How it compares with the other vaginal sets
Our vaginal trays go from a five piece add on to this full set.
| Set | Pieces | What it carries | Buy it when |
|---|---|---|---|
| Major vaginal repair, this page | 63 | Five specula, four retractors, twelve heavy clamps, fistula hooks | The unit does prolapse, fistula or vaginal hysterectomy surgery |
| Vaginal closure | 24 | Mosquito forceps, needle holders, Allis, scissors | Closure and repair work only, with no deep exposure |
| Bartholin cyst excision | 28 | Auvard speculum, Allis, Metzenbaum, artery forceps | The work is on the vulva and vestibule |
| Vesicovaginal fistula repair | 5 | Kelly fistula scissors, micro forceps, probe | You already own this tray and need the fine fistula instruments |
Materials and finish
Every piece in this major vaginal repair instrument set is made from high quality medical grade stainless steel. The steel itself is made to a German specification. We do not publish a grade number. What matters on a Heaney forceps is whether the ratchet still holds a thick pedicle after years of autoclaving, and a number on a certificate cannot tell you that.
The specula and retractors are polished on the blade faces so they slide in without dragging the mucosa, and left satin on the backs to cut glare from the headlight. The heavy clamps have box joints rather than screw joints, which hold their alignment under the load a pedicle puts on them.
Caring for it
With 63 pieces, lay the tray out in the same order every time it is packed. A missing Heaney in a tray this size is easy to miss and hard to work without. Open every ratchet and box joint for washing and the autoclave.
Check the fistula hook prongs and the catheter tip after each case. A bent prong or a burr on the catheter is the kind of small fault that does damage in this field. We straighten, polish and reset; send items back with the item code.
Ordering and wholesale
Every piece of the major vaginal repair instrument set is made by Adas Instruments in our own workshop and sold direct, which is why the price sits where it does against the catalog houses. Order one for a single tray, or in quantity for a hospital list or a distributor line. We do OEM work under your own brand and your own item numbers. Send us a sample and we will hold a run to your pattern rather than ours. Stock items ship from our own workshop. Backorder items take 10 to 12 working days to make. Every instrument carries a three year replacement warranty against manufacturing defects. Production is controlled under an ISO 13485 quality management system.
Frequently asked questions
What is colporrhaphy surgery used for?
To repair a prolapse of the vaginal wall: an anterior colporrhaphy for the bladder bulging in, a posterior one for the rectum. Both are done through the vagina, and both use the instruments in this tray.
What are the names of the specula in this set?
An Auvard weighted speculum, three Doyen specula in two sizes and a Sims vaginal speculum, double ended. Five in all, so the anterior and posterior walls can be held at the same time.
What is a fistula hook used for?
Holding the edges of a fistula or a tissue flap while the surgeon dissects round it. Ours are the Simon pattern, 22 cm, with two prongs.
Can this set be used for a vaginal hysterectomy?
Yes. The Heaney forceps, Museux tenacula and Martin retractors are the core of a vaginal hysterectomy, and all are in this major vaginal repair instrument set.
Does the set include Deaver retractors?
No. It carries Martin, Simon and Gelpi retractors, which suit vaginal work. A Deaver can be added when you order.
