Lighted retractors versus standard blades
A fibre optic retractor puts light at the tip of the tunnel, which is the difference between guessing and seeing when you dissect towards the costal margin. It costs more and needs a cable and source. A plain Deaver or Richardson blade remains the workhorse for the lower flap where the headlight reaches. Many units carry one lighted blade and several standard blades in the same tray.
Clamps for flap traction and fascial plication
Allis clamps grip dermis and fascia firmly with minimal slip, which suits traction on the advancing flap. Kocher clamps bite harder and are better on rectus sheath than on skin you intend to keep.
