Blade length should reach the vallecula or lift the epiglottis without pressing on the upper teeth. Macintosh 3 fits most adult women and smaller men, Macintosh 4 suits larger adults and long necks, and Macintosh 2 covers older children. On the straight side, Miller 0 and 1 serve neonates and infants, Miller 2 covers small children, and Miller 3 is the adult option. Stock one size either side of your usual choice.
Curved Macintosh or Straight Miller
The curved Macintosh blade sits in the vallecula and lifts the epiglottis indirectly, which gives room to pass a tube and is the usual first choice for routine adult intubation. The straight Miller blade lifts the epiglottis directly, which helps in neonates, infants and patients with a long floppy epiglottis or anterior larynx. Departments that intubate across all age groups normally carry both types on every trolley.
Fiber Optic or Conventional Bulb Illumination
Fiber optic blades carry the light source in the handle and deliver it through a bundle, giving even illumination at the tip with no lamp in the mouth and nothing to unscrew during processing. Conventional blades hold a small lamp in a socket on the blade, which is cheap to replace and simple to troubleshoot. Choose fiber optic for high volume theatre use and conventional where field servicing and low cost matter most.

Yes. Blades in this range are reusable stainless steel and fully autoclavable at 134 degrees C. Clean and dry them promptly after use, paying attention to the light channel or lamp socket and the hook-on hinge. Handles are processed with the batteries and battery module removed according to your department protocol, then reassembled and function checked before the set is returned to the trolley.
