What a Halsey needle holder is

The Halsey needle holder is a short fine driver with a straight jaw and a flat, blunt tip, made in one length of 13 cm. It holds small curved needles for delicate closure, and the ratchet locks in the shank so the needle stays put while the wrist turns. It is also sold as a Halsey needle driver and, in some catalogues, as Halsey suture forceps. What separates it from most fine drivers is the choice offered at the tip: plain or serrated, and either of those built left handed.

Plain or serrated, right hand or left

A plain tip grips by flat pressure across a polished face and does not score fine thread. A serrated tip bites, which gives more friction on a slightly heavier needle. On the same 13 cm frame that makes the Halsey unusually flexible: one pattern covers two different jobs depending on which tip is ordered.

The left handed versions are built in mirror image, at the box joint and at the ratchet, not simply handed over to the other hand. A left handed surgeon using a right handed ratchet has to break the lock across the palm, and over a long list that is a real cost. Left handed Halsey needle holders are searched for by name and very few suppliers list them.

Tip options and size

There is one length. The Halsey needle holder 13 cm is also written as 130 mm or 5 inches, and all three describe the same instrument. The choice is at the tip, so this is a tip chart rather than a size chart.

Jaw, carbide and handle

The jaw is straight with a flat blunt tip, short enough to work close to the skin edge without the shank obscuring the field. On the plain build the faces are ground flat and polished; on the serrated build they carry a fine cross pattern. Either way the grip depends on the two faces meeting squarely, and a jaw that has lost that alignment lets the needle rotate.

Two builds are made. A plain steel jaw wears with the instrument. A tungsten carbide needle holder has carbide brazed into the jaw faces, and carbide is harder than the needle, so the face holds its shape far longer and the inserts can be replaced rather than the whole instrument.

Gold plated finger rings are the trade signal for those carbide inserts. Gold rings mean carbide, plain steel rings mean steel. It is a code read across a tray, not a decoration, and our plain steel models never carry the plating.

The body is high quality medical grade stainless steel with a matte anti glare finish, which matters on a fine instrument worked under a strong light at close range. The steel itself is made to a German specification.

Care and reprocessing

Rinse in cool water immediately after use. Warm water sets blood and protein into the box joint, and on a 13 cm instrument that joint is small enough to be difficult to clear once a deposit has set. Clean with a soft brush and a neutral pH enzymatic detergent, work the brush into the ratchet teeth with the instrument open, then rinse with deionized water.

Do not use an abrasive pad on a plain tip. Its whole function is a flat polished face, and a scratched face grips worse, not better. Dry completely, autoclave open rather than locked, and steam sterilize at 134 C for the cycle your unit validates.

Keep the left handed Halsey instruments marked and stored separately. They look almost identical to the right handed ones on a tray, and a surgeon discovering the difference mid closure loses more time than the sorting ever costs.
